4. The Minister of Transport shall, when he has issued the designation under the preceding paragraph, serve public notification to that effect.
(Invalidation and Revocation of Designation)
Article 62-(3). The designation under Article 61 paragraph 1 becomes invalid when any of the following conditions applies to a designated aviation medical institution.
(1)When the validity term attached to the designation in accordance with the provisions of the preceding Article paragraph 3 has expired;
(2)When any operator of a medical institution, etc. who has obtained permission under Article 61 paragraph 1 has to operate the said medical institution, etc.;
(3)When the permission of establishment has been revoked in accordance with the provisions of Article 29 paragraph 1 of the Medical Service Law.
2. The Minister of Transport may revoke the designation under Article 61 paragraph 1, when any of the following conditions applies to a designated aviation medical institution:
(1)When it has violated the provisions of Order issued in accordance with the Law;
(2)When long periods punctuate the medical examinations performed at the institution:
(3)When the medical institution, etc. has been ordered to terminate activities in accordance with the provisions of Article 29 paragraph 1 of the Medical Service Law;
(4)When it fails to satisfy or maintain the standards stipulated under paragraph 2 to 6 inclusive of the preceding Article.
3. The Minister of Transport shall, when the designation becomes invalid in accordance with the provisions of paragraph 1 or the designation has been revoked in accordance with the provisions of paragraph 1 or the designation has been revoked in accordance with the provisions of the preceding paragraph, issue a public notification to that effect.

(Instrument Rating and Flight Instructor Rating)
Article 64. Any person who intends to submit an application for instrument rating or flight instructor rating shall submit the said application for an instrument rating or the said application for flight instructor rating (Form No. 25) to the Minister of Transport.
